This worksheet is a flexible, client-centered tool designed to enhance the therapeutic process during live sessions. It supports both clients and clinicians by providing a structured yet gentle framework for reflection, emotional processing, and focused conversation.
At the start of the session, clients are guided to check in with their current emotional state, thoughts, and body sensations—helping bring the present moment into focus and building emotional literacy. The worksheet also prompts acknowledgment of any progress, no matter how small, which reinforces self-efficacy and builds momentum over time.
Clients then identify their most immediate needs—whether emotional support, problem-solving, regulation, or space to vent—creating clarity around the session’s direction and allowing the therapist to meet them exactly where they are. From there, the client selects a core focus for the session, with optional space to explore triggers or recurring patterns that may have emerged between sessions.
Toward the close of the session, clients reflect on what they’re taking away—whether a shift in perspective, a new tool, or a validating moment. They also identify a small, meaningful intention or action to carry into the week ahead.
For therapists, an optional note section is included to track themes, client strengths, and clinical goals in a cohesive, time-efficient way.
